Grants Application Packet

The application packet consists of the following forms. 

1. Capital Project Grant Application​
2. Affidavit That Funds Will Not Be Used For Religious Purposes
3. Affidavit That Funds Will Not Be Used For Lobbying
4. Certification of Matching Funds (if required)
5. Proof of Insurance (see CGL Booklet for details)
6. Complete the M​aryland Historical Trust Project Review Form​​ and follow instructions at the bottom of the form​. 
7. Include a copy of the Maryland Historical Trust Project Review Form with your Grants Application Packet
   
8. Corporate Diversity Addendum​
